PCN 1D – Changes to our Primary Care Network structure from April 2025
Dear Patients
You may be aware that this practice is part of a Primary Care Network called PCN 1D, this consists of our practice and five others Jai Medical Centre, Mulberry Medical Practice and Oaklodge Medical Centre, Wakemans Hill Surgery and Colindale Medical Centre.
Collectively we have worked to deliver services such as evening and weekend appointments and employed other clinical staff you may have seen – such as Clinical Pharmacists and First Contact Physiotherapists.
What is changing?
From April next year, there will be a slight change in this structure. PCN 1D will change into two PCNs – Oak Lodge Medical Centre and Mulberry Medical Practice will become PCN 7, and Colindale Medical Centre, Hendon Way Surgery, Jai Medical Centre and Wakemans Hill Surgery will become PCN 1D.
What does this mean for me?
There will be no changes in the way you currently access your practice or services.
Why is this happening?
All practices have agreed that splitting into two PCNs will be better for patients going forwards, it will allow each smaller PCN to coordinate and deliver services that are appropriate and bespoke to meet the needs of our individual patient cohorts.
